Time Commitment: 5 hours per week
Job Summary:
The Link Health Remote Patient Navigator assists individuals and families in navigating and enrolling in government assistance programs such as LIHEAP, Supplemental Nutrition Assistance Program (SNAP), Lifeline, and other state/federal benefits as a way to address social determinants of health. This role combines social services, client support, data management, and public assistance knowledge with components of health equity.

Link Health actively seeks to assist eligible people in navigating and enrolling in benefit programs that address crucial needs such as affordable internet access, food access, healthcare support, and housing resources.
We use community-centered approaches that leverage data, technology, and partnerships. This reduces barriers and connects underserved populations to the benefits available to them for improved economic stability and health outcomes.
Duties & Responsibilities:
- Commit to 20 hours per month of remote shifts for the 16-week cohort (approximately 5 hours per week, scheduled as two 2-hour shifts twice per week plus one hour in team huddles).
- Screen patients remotely to understand each client's household, income, expenses, and eligibility factors for various benefit programs via text message, chatbot, or Link Health dashboard..
- Complete Lifeline applications end-to-end, including submission via the FCC website (MA) or state-specific in-clinic process (TX).
- Assist clients in accurately completing program applications, using the online portal or Link Health’s enrollment process.
- Follow up on enrolled cases at 24-48 hours, 1 week, and 2-3 weeks to confirm benefit access.
- Process chatbot intake and escalate unclear cases to the state-level Senior Patient Navigator
- Track all activity and enrollment metrics in the Link Health dashboard
- Ensure all client interactions & files remain compliant with privacy & security protocols.
- Communicate professionally and appropriately with the leadership team and Senior Patient Navigators
- Complete all required compliance documentation
- Opportunity to write and publish op-eds, conduct research, and present at conferences
