Remote Prescribing Pharmacist
Description
This role combines core Remote Prescribing Pharmacist responsibilities with dedicated delivery of the North West London Cardio-Renal Metabolic (CRM) Enhanced Service, giving the postholder a varied caseload split between general remote consultations/repeat prescribing governance and structured long-term condition care planning for patients with, or at risk of, cardiovascular, renal and metabolic conditions.
The role suits a pharmacist who wants breadth across day-to-day digital primary care pharmacy work as well as depth in a specific long-term condition programme. Time will typically be split between general remote prescribing workstreams (desktop reviews, QOF reviews, medication reviews, acute condition presentations) and CRM-specific appointments.
The postholder is line managed day-to-day by the Workforce Lead Pharmacist, covering performance, conduct, wellbeing and workforce development. In addition, the postholder receives clinical input, training and quality oversight from the Medicines Management Lead Pharmacist and the Prescribing Lead Pharmacist across both the general prescribing and CRM elements of the role.
Please note: This is a remote role, working 20hrs or more a week, from Monday to Friday, between 8am to 8pm.
WHAT YOU WILL WORK ON:
- Remote Prescribing and Consulting
- Conduct remote consultations, structured medication reviews, and long-term condition support, with thorough risk assessment and appropriate safety netting.
- Conduct desktop medication reviews to reauthorise repeat prescription templates, and triage/action prescription medication requests, checking for red flags before issuing, declining or escalating.
- Review and action documents, carrying out medicines reconciliation and issuing prescriptions or coding as appropriate.
- Deliver core QOF reviews (statin initiation, aspirin initiation, CHA₂DS₂-VASc/AF, CKD annual reviews) and support EPC contract requirements where these fall outside dedicated CRM appointments.
- Cardio-Renal Metabolic Appointments
- Deliver Discussion appointments: 30-minute structured Health and Wellbeing care-planning conversations using motivational interviewing/health coaching, covering the six pillars of lifestyle medicine, with onward referral to social prescribing and community support where appropriate.
- Deliver Follow-Up appointments to review progress against agreed goals and administer the Health Confidence Score.
- Optimise hypertension, diabetes and CKD medicines in line with NICE guidance and CRM-specific BP/SGLT2/RAAS-blockade targets.
- Ensure accurate coding to support CRM01–09 outcome indicators alongside core QOF coding, using the NWL CRM template and Dashboard to track delivery.
- Other Responsibilities
- Collaborate with multidisciplinary teams, including GPs, nurses, social prescribing link workers, health and wellbeing coaches, and voluntary/community sector partners.
- Maintain accurate, structured, and timely clinical records across both workstreams, ensuring notes are appropriate in length and easy to read for colleagues working either caseload.
- Contribute to quality improvement initiatives and audits spanning both general prescribing safety and CRM outcome achievement.
- Engage constructively with feedback, coaching and quality reviews from the Medicines Management Lead Pharmacist and Prescribing Lead Pharmacist, and escalate workforce or wellbeing matters to the Workforce Lead Pharmacist.
- Compliance and Professional Development
- Engage in CPD relevant to both general remote prescribing practice and long-term condition/CRM management.
- Complete CRM competency framework training (motivational interviewing, health coaching, CRM condition management) alongside standard onboarding for digital remote prescribing.
