About the Role
We’re looking for a Renal Care Coordinator to support patients across KidneySPA centers, with a focus on helping people adjust during their first 90 days of dialysis and stay connected to care after a hospitalization. You’ll work with patients, families, facility teams, and external care partners to support communication, follow-up, and the patient experience.
This multi-site role also supports assigned quality improvement, EMR implementation, and workflow projects. It is a coordination role; clinical questions and decisions are referred to licensed professionals.
What You’ll Do
- Follow up with newly admitted dialysis patients during their first 90 days and help them navigate nonclinical questions and service needs.
- Check in on treatment attendance and patient-reported barriers, then connect patients with the appropriate team members.
- Help coordinate communication and follow-up during hospitalizations and after discharge.
- Work with facility teams and care partners to support return-to-dialysis arrangements.
- Document outreach and follow-up activities in approved systems.
- Support quality improvement, EMR implementation, and workflow projects across KidneySPA locations.
