Residental QIDP Case Manager

Residential QIDP (Community Living Services)

Full-Time | Exempt | Flexible Schedule

Participation in on-call rotation required

As the QIDP, You Will:

  • Provide person-centered case management by developing, implementing, and monitoring individualized service plans that promote independence, dignity, and quality of life for individuals with intellectual and developmental disabilities (IDD).
  • Oversee Host Home providers and residential services by completing site visits, environmental audits, assessments, and ensuring individuals receive high-quality support that meet agency and DHS standards.
  • Coordinate medical, behavioral health, psychiatric, Community Day Services, and other community-based supports to ensure each individual's needs and preferences are met.
  • Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.
  • Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care.

Education/Experience Requirements

  1. Bachelor’s Degree preferred in Human Services or education and two years’ supervisory experience required.

  1. Supervisory experience is essential.

  1. One year experience with persons with developmental disabilities.

  1. The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence.

  1. Ability to effectively present information in one-on one and small group situations to customers, clients, and other employees of the organization.

  1. Frequent walking and sitting.

  1. Regular reaching, grasping, and standing.

  1. Ability to lift 50 lbs.
