About Us

Quality Healthcare Group, Inc. (QHCG) is a CARF-accredited human services organization based in Delaware County, Pennsylvania, providing home health and community-based services to individuals across the lifespan. We are committed to delivering person-centered, high-quality care that promotes independence and quality of life for the individuals we serve. Our services span Title 28 home health (Chapters 601/611), residential habilitation, and intellectual/developmental disability (IDD) support under Title 55 (Chapters 6100/6400), as well as OLTL/Community HealthChoices and ODP waiver programs.

Position Summary

We are seeking a skilled and mission-driven Registered Nurse to serve as Nurse Manager for QHCG's residential habilitation and IDD service programs. The Nurse Manager is the primary clinical authority across QHCG's residential homes and community-based sites, responsible for the health and safety of all individuals served, clinical oversight of direct support professionals, medication management systems, regulatory compliance under Pennsylvania Title 55, and coordination with physicians, specialists, families, and interdisciplinary teams.

This role also carries admissions and intake responsibilities, serving as a clinical gatekeeper for new referrals from hospitals, MCOs, and Support Coordinators — making it a high-visibility, operationally significant position within the organization. The ideal candidate is a composed, detail-oriented clinical leader with direct experience in IDD, group home, or residential care settings and a working knowledge of Pennsylvania ODP regulatory standards.

Key Responsibilities

Clinical Oversight & Health Management

  • Conduct and document health assessments, medication reviews, and ongoing nursing monitoring for all individuals receiving residential and community-based supports
  • Develop, maintain, and update individualized health-care plans addressing chronic conditions, risk factors, preventive care needs, and post-hospitalization follow-up
  • Oversee medication management systems across all residential sites, including medication storage, administration protocols, MAR accuracy, and error identification and response
  • Ensure timely implementation of physician orders and continuity of care following hospitalizations, specialist visits, or changes in health status
  • Manage infection control protocols, health-related incident reporting, and nursing-level risk mitigation across all service locations

Admissions & Intake

  • Receive, review, and triage incoming referrals from hospitals, MCOs, case managers, and community partners
  • Conduct preliminary clinical assessments within RN scope of practice and escalate complex cases to the Clinical Director or Administrator as appropriate
  • Verify insurance and MCO eligibility, service codes, prior authorizations, and required payer documentation prior to admission
  • Coordinate collection of medical records, medication lists, PCP notes, hospital discharge summaries, and supporting clinical documentation
  • Communicate with prospective residents, families, and guardians to explain services, gather intake information, and facilitate start-of-care logistics
  • Ensure all admission paperwork is complete, accurate, and compliant with DHS/OLTL/ODP regulations and QHCG policy
  • Collaborate with scheduling, operations, and program leadership to confirm appropriate staffing prior to accepting a new case
  • Maintain detailed admission logs, timelines, and documentation trails for internal auditing and regulatory compliance

Regulatory Compliance & Quality

  • Maintain nursing practices fully aligned with Pennsylvania Title 55 Chapter 6400 requirements, including medication administration oversight, health services documentation, and staff competency standards
  • Ensure compliance with Chapter 59 program standards where applicable to QHCG's service model
  • Support CARF International continuous quality improvement activities, including outcome measurement, survey preparation, and corrective action planning
  • Maintain agency readiness for ODP monitoring visits, DHS licensing inspections, and CARF accreditation surveys
  • Participate in QAPI initiatives, trend analysis, incident review, and health-related performance improvement activities

Staff Training & Supervision

  • Train, delegate to, and evaluate DSPs and residential staff performing health-related duties and medication administration
  • Conduct and document competency testing for medication administration; implement retraining when errors or gaps are identified
  • Provide ongoing coaching and guidance to ensure staff understand and correctly execute medical protocols, emergency response procedures, and person-centered health support practices
  • Identify training gaps and develop targeted in-service education in coordination with program leadership

Coordination & Communication

  • Serve as the primary clinical liaison with physicians, pharmacies, hospitals, specialists, Support Coordinators, and families/guardians
  • Participate in interdisciplinary team meetings, ISP planning conferences, and transitions-of-care discussions
  • Communicate health status changes, emerging risks, and significant medical events to QHCG leadership and residential support teams in a timely and documented manner

Documentation & Records Management

  • Maintain secure, accurate, and thorough medical documentation consistent with Pennsylvania legal and regulatory requirements
  • Review MARs, health logs, nursing notes, incident reports, and appointment records for accuracy, completeness, and appropriate follow-through
  • Track immunizations, annual health screenings, specialist appointments, and preventive care schedules for all individuals served
  • Utilize Therap or other designated EHR platforms for clinical documentation, health tracking, and compliance reporting

Quality & Safety

  • Lead nursing-level investigations and corrective action plans following serious incidents, medication errors, or unexpected health events
  • Monitor environmental safety conditions related to infection prevention, medication storage, and residential site health standards
  • Support organizational quality improvement efforts through trend monitoring, data collection, and performance reporting aligned with CARF frameworks

Required Qualifications

  • Active, unrestricted Pennsylvania Registered Nurse (RN) license in good standing
  • Minimum 2–3 years of clinical nursing experience, with at least 1 year in an IDD, group home, residential habilitation, or behavioral health setting
  • Working knowledge of Pennsylvania Title 55 Chapter 6400 regulations and nursing delegation principles in residential care
  • Strong documentation skills and demonstrated ability to interpret and implement physician orders and health data

  • Ability to pass Pennsylvania criminal background check (Act 34), child abuse clearance (Act 151), and FBI fingerprinting (Act 114)
  • Valid Pennsylvania driver's license and reliable transportation for inter-site travel
  • Current CPR/BLS certification

  • Familiarity with Therap EHR platform
  • Prior supervisory or nurse-lead experience in a residential or community-based setting
  • Knowledge of ODP waiver programs, OLTL/Community HealthChoices service frameworks, and HCSIS/EIM incident reporting
  • Experience with admissions, intake coordination, or discharge planning in a regulated care environment
  • Bilingual candidates encouraged to apply

Compensation & Benefits

  • Competitive salary commensurate with experience and qualifications
  • Full-time salaried position with clinical leadership-level compensation
  • Supportive, family-owned leadership team with a strong compliance and quality culture
  • Opportunity to build and shape QHCG's clinical infrastructure as the organization continues to grow
