Residential Qualified Mental Health Professional
Swope Health is seeking a passionate and mission-driven Qualified Mental Health professional(QMHP) to provide high-quality support and services to our residents within the Swope Health residential facilities. The QMHP directs clinical services for all residents focusing on the unique needs and goals of our residents. Primary Duties:
- Perform person centered clinical assessments with residents to determine level of education, monitoring, and support required within the residential setting.
- Coordinate treatment planning services in conjunction with the CPRP and Healthcare Home team to assess and monitor individual progress, adjustment to community living, participation and progress in organized treatment programs, educational or vocational programs, and community participation to assure the individual receives the best care in the least restrictive environment possible.
- Lead the coordination of recovery efforts for individuals with mental illness, chemical dependency and/or co-occurring disorders in a manner that supports the dignity, respect self-determination and empowerment of each individual.
- Provide ongoing training and coaching of intensive residential specialists, mental health technicians and aides, and peer support staff relative to their job objectives, standards of performance, clinical best practices and individual development needs.
- Maintain clinical and administrative documentation according to the standards set by Swope Health Services and regulatory entities, to include electronic health records, billing documentation, and residential program outcome reports.
- Participate in assigned Quality Improvement, Accreditation and Standards, and Risk Management activities and
meetings. - Conduct weekly quality assurance activities including but not limited to a review of written documentation to ensure the quality and appropriateness of service delivery and validation for billing, frequent onsite residential checks to ensure satisfaction with services and compliance with the standards of Swope Health Services and other regulatory entities.
- Training, coaching and supporting in daily living skills, including housekeeping or daily chores, cooking, personal grooming, accessing transportation, keeping a budget, paying bills, functioning at work and in the classroom and initiating and maintaining an independent residence or maintaining residence in a family setting.
- Assisting in creating personal support systems that include work with family members, legal guardians or significant others regarding the needs and abilities of an identified person.
- Encouraging and promoting recovery efforts, consumer independence/self-care and responsibility.
- Providing support to families in areas such as treatment planning, dissemination of information, linking to services
and parent guidance.
Education/Qualifications
- Masters Degree in Social Work, Nursing, Counseling or related field. Clinical license in the State of Missouri preferred.
- A qualified mental health professional with two years’ experience in the mental health field, at least one of those years working with the seriously and persistently mentally ill.
- Ability to manage complex personnel and community partnership issues with professionalism, diplomacy and tact.
- Strong interpersonal skills, excellent verbal and written communication skills.
- Strong consumer advocacy skills and empowerment oriented.
- Ability to effectively prioritize and manage simultaneous task.
