PCSP/IEP PROCESS PARTICIPATION
- Advocate for members to ensure their best interests are held foremost in all decisions and actions directly affecting their lives.
- Serve as primary contact to members, parents/guardians, employees, and other agencies to provide information and support to ensure the members' needs are adequately met.
- Assist with intake procedures to involve the collection of medical and psychological records, consent and release of information, and any other financial/legal documentation required to permit the member to enter AZTEC program(s).
- Assist in initial planning to involve assessment of current skills and placement in the most appropriate program area(s).
- Orientate members to facilities, facilitate the development of PCSP/IEP, collect and dispense information, facilitate family/guardian contact, and coordinate all correspondence/requests to/with DES-DDD.
- Schedule, coordinate, facilitate, and obtain a copy of the document for all PCSP/IEP meetings.
- Ensure all assigned components of the Annual PCSP/IEP are completed for presentation to the member's team.
- Complete the Consumer Satisfaction/Quality of Life Survey form(s).
- Follow along and advocate for member services within the program(s) and for those who have matriculated through AZTEC.
SUPPORT SERVICES
- Maintain and update complete, accurate, and current information in Confidential Files in accordance with DES-DDD Licensure and AZTEC Administrative Policies.
- Review and provide follow-up on Incident Reports. Report all Serious Incidents (SIR) to appropriate AZTEC personnel in a timely manner.
- Responsible for prompt reporting of all Hospital Admissions, Emergency Physical Intervention Techniques (Prevention & Support), and Serious Incident Reports to DDD Central IR/West IR within designated timelines.
- Responsible for updating and maintaining all forms in the electronic lists section and distributing (as applicable).
- Monitor ledgers to ensure adequate funds are available. Request checks/ funds to purchase needed items and services for members.
- Maintain and update Member Inventory Form.
- Process all incoming/outgoing mail on a daily basis, including all monthly bills, i.e., cable, cell phone, and medical (as applicable), and ensure they are paid in a timely manner.
- Responsible for coordinating home visits and recreational events, including creating a list for the Scheduling Department.
- Complete Exit/Discharge Summary and Follow-up Report upon the termination of services.
- Complete Birthday Check Requests in an expedient manner and ensures that gifts are wrapped and delivered.
- Ensure each member's birthday is celebrated with housemates, family members, and friends as desired.
- As assigned, responsible for cost comparing, coordinating, and/or purchasing of member's personal belongings and for submitting receipts to the Business Office within the required twenty-four (24) hour timeline.
HEALTH/MEDICAL SERVICES
- Advocate for all members to ensure needed medical services/appointments are obtained in a timely manner and confirmed in advance.
- Ensure all medical referrals and lab appointments are scheduled in advance, noted on Electronic Calendar, and processed in a timely manner.
- Review completed paperwork to ensure the completion of follow-up recommendations made by physician/specialist for all evaluations, routine medical appointments, and ER/Urgent Care and hospitalizations.
- Responsible to complete medical emergency paperwork, fill/start prescriptions/procedures, submit paperwork in a timely manner to appropriate personnel, and communicate follow-up and end-of-shift updates via email and/or voicemail.
- Monitor the accuracy and completion of required hospital and/or medical/dental consents, and ensure they are received prior to all procedures being completed.
- Ensure medical paperwork is legible. Seek clarification from a physician/specialist when necessary. Complete a Progress Note as needed. Ensure documents are signed by the physician/specialist before processing.
- Provide or arrange for transportation to medical appointments and facilities, as needed, especially in case of emergency.
- Accompany members to the hospital and complete needed paperwork/follow-up during designated hours and in cases of medical emergencies, as needed.
- Update nutritional/diet sheet and special needs when a dietary recommendation is made.
- Document periodic weights, as requested by the physician.
- Ensure, through delegation, monitoring, and/or completion, that Emergency Medical Procedures are followed. Provide prompt notification to AZTEC management personnel.
- Prepare necessary paperwork for all required documents (i.e., Behavior Plan, Behavior Graph, and/or food logs, co-pays, consents, lab/procedure orders, I.D., insurance cards, etc.).
- Request funds and/or co-pays in advance.
- Coordinate and monitor the completion of repair/replacement of adaptive equipment through the Primary Care Physician (PCP) and Durable Medical Equipment Provider (DME) Provider.
- Notify the physician/specialist if any medical problem or change in health occurs.
- Obtain and document blood work results or other dictation from laboratories or physicians and communicate results, in writing, to the Health Services Director.
- Document telephone calls with physicians/specialists and canceled appointment/lab forms on Progress Notes for inclusion in members' Confidential Files.
- Ensure medical insurance changes, updates, and/or information is updated for inclusion in the member's Confidential File and scanned into the member's electronic records.
- Complete Residential Activity forms daily for face-to-face and on-behalf of meetings. Submit to the Residential Support Coordinator in a timely manner.
- Prior to leaving the work shift, ensure that all necessary paperwork for appointments is submitted to the Residential Support Coordinator.
- Check and respond to emails throughout the day.
- Check and respond to voicemail messages. Telephone calls should be returned within 24 hours.
- Complete Medical Referral Report forms include checking the box which indicates whether or not the appointment is for ER/UC follow-up. Include date/s as applicable.
PROGRAMMING
- Complete and file Monthly Training Progress Reports within ten working days of the scheduled due date and distribute them to the DDD Support Coordinator and guardian (as applicable).
- Schedule, prepare, and attend all psychotropic medication reviews, ensuring compliance with DES-DDD Licensure and Article 9.
- Update all Psychotropic Consents in accordance with prescribed DES-DDD and Article 9. Ensure all medication increases/ decreases are implemented in a timely manner.
- Report the use of emergency physical management measures in accordance with DES-DDD Licensure and Article 9.
MANAGEMENT/SUPERVISORY
- Serve as primary supervisory contact for residential employees.
- Ensure, through delegation, monitoring, and/or completion, that all medical procedures are followed and residential employees are fully informed of the same.
- Facilitate positive and professional direct-support interaction with members.
- Conduct employee orientations to members and their residential settings as assigned.
- Identify residential employees and become mentors for Orientation to members and their homes.
- Re-orientate employees to the members/residential setting for fill-in, transferred, and on-call employees during their scheduled work shifts.
- Return telephone calls in a timely manner (within fifteen minutes).
- Provide employees with training and monitoring sufficient to enable them to function effectively.
- Provide appropriate, professional, innovative, and meaningful leadership.
- Establish proper professionalism on the part of employees supervised.
- Maintain communication between residential and administrative employees.
- Utilize established AZTEC procedures in order to correct/document unacceptable work performance by employees supervised.
- Maintain documentation of daily observed performances of residential employees as appropriate.
- Provide input to Human Resources Manager on performances and response to concerns by employees following AZTEC Employee Handbook and AZTEC Administrative Policies.
- Monitor adherence to menus/diets/diet sheets by residential employees, and as assigned, complete grocery shopping for assigned homes.
- Ensure members participate in a variety of recreational outings, hobbies, and leisure activities on a weekly basis, as outlined on the Recreation Calendar.
- Responsible to read, initial, and respond daily to all communication books for each residential setting assigned.
- Monitoring of recreational funds for accuracy and for submittal of receipts to the Business Office.
- Understand and maintain residential settings facilities in accordance with DHS Licensure.
- Report needed repairs and for assisting in the coordination of repairs and maintenance within the residential settings.
- Complete and submit fire drill evacuations and safety checklists in a timely manner.
- Attend and facilitate DES-DDD Licensing/Monitoring Reviews in residential settings.
- Monitor and provide residential settings with necessary toiletries, cleaning supplies for members, etc.
- Assist with supervision during transportation and assist daily with ensuring transportation is completed and medication boxes and crates are picked up.
- Ensure residential vehicles are washed and cleaned, inside and out, on a weekly basis.
- Ensure van checks are completed monthly, checking vans for first aid supplies, proof of insurance, registration, directions to AZTEC settings, and accident information forms.
- Ensure AZTEC vehicles are fueled two times weekly.
- Ensure proper safety techniques and procedures are utilized and followed.
ADDITIONAL DUTIES
- Read, understand, and abide by all AZTEC Administrative Policies, AZTEC Employee Handbook, AZTEC Job Description, DES-DDD, DHS Licensure, and any other local, state, or federal regulations pertaining to the job. This includes timely notification of violations to supervisor(s).
- Fill in as an Instructor in order to maintain ratios in an emergency situation.
- Assist AZTEC in maintaining open, honest communications with the public concerning program goals and philosophy.
- Aware of and follows basic safety skills to avoid injury of self or others. Report and follow up, as necessary, on unsafe conditions that could potentially result in injury. Utilize proper lifting techniques as presented and demonstrated in AZTEC training. Monitor compliance with safety standards on part of employees supervised, addressing remediation of noncompliance issues in a timely manner.
- Maintain current Arizona Class D driver's license and satisfactory motor vehicle record. Notify Human Resources Department immediately of any infractions, revocation/suspension, etc., affecting driver's license and/or motor vehicle record. Drive safely, abiding by AZTEC policies and Arizona Department of Transportation laws.
- Strict adherence to the AZTEC Secrecy Agreement, Confidentiality Policy, and Acknowledgement Regarding Employment. This includes notification to the supervisor of known or suspected violations.
- As requested, assist AZTEC management with input regarding performance evaluation of employees within the area of specialty.
- Maintain work area in a neat, clean, and very tidy manner. This includes, but is not limited to, the top of the desk, desk drawers, and all personal objects around the work location.
- Responsible during fire evacuation drill, or in the event of a real fire, for immediately evacuating in a calm and quick manner, per AZTEC evacuation procedure. This will include assisting members to evacuate.
- Consistently exhibit professional behavior and provide a favorable impression as a representative of AZTEC.
- Responsible for immediately reporting any arrest to the Human Resources Department prior to working any work shifts after the arrest.
- Responsible for immediately reporting to the Human Resources Department any traffic violation or ticket received, whether on duty or off duty, and any driver's license suspension/revocation.
- Responsible to report in a timely manner any change in employee status (i.e., address change, telephone change, name change)
- Assist in monitoring wheelchair lift operations during transportation arrival and departure at the Adult Services Program
- Additional duties as required or assigned.
RATE OF PAY: $19/HR
