Summary

A part-time rheumatologist evaluates, diagnoses, and treats autoimmune and rheumatic conditions, performs procedures, and coordinates care with primary-care teams in an integrated healthcare setting.


PRIMARY PURPOSE:

The Rheumatologist is responsible for providing comprehensive, patient-centered evaluation, diagnosis, and treatment of rheumatic and autoimmune conditions within FoundCare, Inc.’s community-based, integrated healthcare model. This role focuses on delivering high-quality, evidence-based specialty care to a diverse and often underserved patient population, collaborating closely with primary-care providers and care coordination teams to improve patient outcomes. The Rheumatologist supports FoundCare’s mission by promoting access to specialty services, advancing continuity of care, and contributing to quality improvement initiatives, patient education, and population health efforts in an inclusive, compassionate clinical environment.



ESSENTIAL JOB FUNCTIONS:


Clinical Duties

  • Evaluate, diagnose, and manage patients with rheumatic diseases such as rheumatoid arthritis, lupus, gout, vasculitis, spondyloarthropathies, and osteoarthritis.
  • Interpret diagnostic tests including laboratory studies, imaging, and specialized procedures (e.g., joint aspiration, synovial fluid analysis).
  • Perform in-office procedures such as joint infections, tendon sheath injections, and aspirations.
  • Develop individualized treatment plans using medications, lifestyle interventions, and coordinated care.
  • Monitor disease progression and adjust therapies based on clinical response and evidence-based guidelines.


Patient Education & Support

  • Counsel patients on disease processes, treatment options, medication risks/benefits, and long-term management strategies.
  • Promote self-management skills and shared decision-making.
  • Collaboration & Coordination
  • Work closely with primary-care physicians are other specialists.
  • Participate in multidisciplinary case reviews and care planning.


Documentation & Compliance

  • Maintain accurate and timely medical records.
  • Ensure compliance with regulatory, ethical, and institutional standards.
  • Utilize electronic health record (EHR) systems effectively.
