Position Summary:
As the RN Care Coordinator, you would be responsible for providing case management and outreach services to assigned cases.
Hours for this Position: Monday-Friday 8am-5pm

More Insight of Daily Responsibilities:
- Complete assessments and develop/implement appropriate care plans based off of findings
- Provides information regarding benefits and community resources available
- Identifies potential risks and barriers
- Communicates and coordinates with family members, physicians, and other healthcare professionals
- Completes face-to-face visits in order to monitor changes and track progress
What We Look For:
- Clear and active RN license in state of IL
- 3+ years of Case or Disease Management experience
- Able to travel locally (must have valid driver's license and auto insurance)
- Pediatric experience preferred
