AdventHealth will bring a 60-acre health and wellness campus to Lake Nona in late 2026. This location will offer medical innovations on a campus designed around whole person health with comprehensive services, destination lifestyle programs, and much more that will shape the next generation of health care.

We are proud to be a part of the growing Lake Nona community with an established comprehensive network of care. Our services include emergency services, urgent care, a wide range of medical specialists (including primary care), lab, imaging, orthopedics, sports medicine and rehabilitation.

  • Monitors the recovery of discharged patients through the Patient Call Back Program.
  • Calls discharged patients at designated times to ensure patient satisfaction and compliance with discharge education.
  • Assesses patient knowledge, compliance, and understanding of discharge instructions through outbound calls.
  • Identifies gaps or barriers in the discharge treatment plan and communicates effectively with the multi-disciplinary team.
  • Provides and reinforces patient and family education as planned by the discharge team.

Knowledge, Skills, and Abilities:

  • Excellent communication and interpersonal skills [Required]




Education:

  • Associate [Required]




Field of Study:

  • Graduate of an accredited school of nursing




Work Experience:

  • 3+ as a licensed vocational nurse [Required]
  • Basic level of experience with Microsoft Word and ability to navigate a Windows environment. [Required]
  • Skill in assessing, planning, implementing and evaluating patient care as acquired through clinical nursing [Required]



Licenses and Certifications:

  • Registered Nurse (RN) [Required]
