Big Bend Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our interdisciplinary hospice care team. In this rewarding role, you will coordinate and provide high-quality end-of-life care, helping patients and families achieve the best possible quality of life through expert symptom management, education, advocacy, and support.
Position Summary
The RN Case Manager develops, coordinates, and implements the interdisciplinary plan of care for assigned hospice patients across various care settings. This role is responsible for patient assessments, care planning, symptom management, patient and family education, and collaboration with physicians and the hospice interdisciplinary team. The RN Case Manager also supervises Hospice Aides and LPNs while ensuring compliance with hospice regulations, nursing standards of practice, and Big Bend Hospice policies and procedures.
Key Responsibilities
- Manage a hospice caseload of approximately 18–25 patients.
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Coordinate and oversee care delivered by the interdisciplinary team.
- Provide expert pain and symptom management.
- Educate patients, families, and community partners regarding hospice care.
- Maintain timely and accurate documentation within the electronic medical record (EMR).
- Collaborate with physicians and interdisciplinary team members to ensure quality care.
- Participate in quality improvement initiatives and interdisciplinary team meetings.
- Assist with orientation and mentoring of new staff and students
Work Schedule:
- Monday-Friday; 8:00 am- 5:00 pm; Rotating Holidays
Travel Requirement: You must have a reliable vehicle and be willing to be dispatched throughout our service areas.
