RN Case Manager (Central)
As an RN Case Manager, you'll manage your own patient caseload, coordinate care across an interdisciplinary team, and help patients recover, manage chronic conditions, and remain safely in the comfort of their homes. What You'll Do
- Manage a caseload of home health patients, providing skilled nursing care and comprehensive case management.
- Conduct Start of Care admissions, comprehensive assessments, and ongoing evaluations to identify patient and family needs.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary care team.
- Coordinate nursing, home health aide, therapy, and other support services to ensure high-quality, seamless patient care.
- Monitor patient progress, communicate changes in condition, obtain physician orders as needed, and adjust care plans accordingly.
- Educate patients and caregivers on diagnoses, medications, treatments, and self-management strategies to promote independence.
- Complete timely, accurate clinical documentation in accordance with agency, state, and Medicare guidelines.
- Participate in discharge planning and ensure smooth transitions when care goals have been achieved.
What You'll Bring
- Graduate of an accredited School of Nursing.
- Current Registered Nurse (RN) license in Connecticut or Compact RN License.
- At least one year of nursing experience.
- Strong clinical assessment, critical thinking, communication, and organizational skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Home health experience and OASIS documentation experience are a plus but not required.
