We are actively expanding our presence in Jackson and surrounding communities and are seeking a compassionate and skilled RN Case Manager to help us build and strengthen patient care throughout this growing territory.

• Brooklyn

• Chelsea
• Ann Arbor
• Ypsilanti
• Saline
• Manchester
• Spring Arbor
• Parma
• Concord (as needed)

This role is ideal for an RN who values autonomy, strong clinical judgment, and the opportunity to grow alongside an expanding patient census.

In This Role, You Will

• Conduct comprehensive nursing assessments
• Develop individualized plans of care
• Deliver skilled nursing interventions in the home
• Educate patients and families on condition management and medications
• Coordinate with physicians, therapists, and interdisciplinary team members
• Complete accurate and timely documentation in WellSky / Kinnser
• Utilize Home Health Notify for real-time communication and updates
• Ensure compliance with state, federal, CHAP, and Joint Commission standards
