RN Case Manager, MAT

The RN Case Manager, MAT assesses and monitors the client's physical and psychological withdrawal from mood-altering chemicals and administers medications as prescribed with the goal of ensuring the client detoxifies safely.

Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services needed to meet an individual’s health needs, using communication and available resources to promote quality and cost effective outcomes.

The RN Case Manager, MAT will provide quality patient care via the development, implementation & evaluation of individual patient care plans. (S)he is a liaison between the patient, the family of the patient, community partners, and organization-wide within WSMC.
