RN Case Manager Private Care (Temporary)

Assume responsibility for the primary needs of a group of clients, including assessment of client and family, care plan development and oversight for the duration of service, and coordination and collaboration with the health care team. Oversee the care given by RN, LPN, CNA/HHA and Companion staff. Incorporate and demonstrate the Core Values of the VNA – Integrity, Collaboration, Accountability, Respect, and Excellence at work and whenever representing the VNA. Follows regulatory requirements and mission and vision of VNA of the Treasure Coast.
Responsibilities:

Provide skilled nursing services in accordance with the orders of the physician.
Conduct initial assessment and establish plan of treatment. Obtain thorough physical, psychosocial and environmental data. Develop and follow a care plan which identifies problems (nursing diagnosis) and evaluates outcomes within a specified timeframe.
Assume primary responsibility for a client caseload. Coordinate all services and collaborate with members of the health care team.
Provide ongoing assessment of client response to treatment, medication and teaching. Provide interventions and revise as needed.
Communicate with physician to confirm, correlate, update client’s plan of treatment and provide physician with a status report on a regular basis.
Participate in chart audits and QA activities.
Attend case conferences with supervisor on a regular basis to discuss new admissions, potential discharges, clients due for recertification and problem solving.
Provide instruction and supervision of RN, LPN and home health aide and companion staff on a timely basis; and in accordance with agency, State and Federal regulations.
Collaborate with the Administrator to complete and deliver 90 day, quarterly and annual performance feedback and evaluations for Home Health Aides and Companions.
Utilize office and work time efficiently and perform in an organized manner.
Use, store and maintain equipment and supplies in a safe, appropriate manner.
Assist in orientation of new employees and students to Private Care , including instructing staff on orientation, skill training, testing, and any other training, e.g., soft skills, in-service workshops or other topics related to providing appropriate care to clients. Evaluate competence of staff after training and using evaluation tools (90-day, competency, shared visit, annual evaluation) established by the agency or Human Resources.
Monitor new clinical staff during the orientation period, including making home evaluation visits and documentation oversight.
Utilize inter-agency and community resources to assure continuity of care.
Follow HIPAA guidelines and maintain all protected health information (PHI) obtained in the course of work as confidential and, as such, should not be a matter of discussion with co-workers or others outside of the Agency.
Attend mandatory staff meetings, team meetings and in-service programs. Fulfill mandatory e-learning courses by the due date.
Perform all nursing duties in accordance with the Florida Nurse Practice Act and comply with VNA policies and procedures.
Maintain and update client records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Follow VNA safety policies/procedures. Report accidents or unusual incidents immediately to the immediate supervisor and Human Resources.

Qualifications:

Active RN license in the State of Florida
Current Basic Life Support for Health Care Provider Certification/CPR
Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing
Current and valid Florida driver’s license
Current automobile insurance per VNA requirement
