Description
The Good Shepherd Inpatient Rehabilitation Unit is comprised of an interdisciplinary team of qualified professionals who meet the special needs of each patient. Our entire clinical staff works as a coordinated team to ensure that patients receive the greatest benefit from the rehabilitation process. This approach aims to effectively address all aspects of life that have been affected by illness or injury. We employ expert physical, speech and occupational therapists to provide at least three hours of therapy five days per week.
The Case Manager is responsible to the Director of Case Management for the coordination of care of a selected group of patients to achieve optimal outcomes with effective resource utilization from admission through discharge. The Case Manager exhibits a leadership role of influence, collaborates with interdisciplinary team members both internally and externally, and ensures continuous, efficient, quality care.
