RN Chronic Care Case Manager (30 Hours/Week)

RVNAhealth is seeking a compassionate Registered Nurse to support older adults and individuals with chronic health needs throughout Western Connecticut. This unique role is ideal for an RN looking for flexibility, meaningful patient relationships, and the opportunity to make a lasting impact in the community.
Unlike traditional home health nursing, this position focuses on ongoing wellness visits, general health assessments, medication pre-pours, care coordination, and patient support-not complex skilled nursing care.

Territory
This role supports patients throughout Western Connecticut, from Sherman to Stamford. While the territory is broad, schedules are thoughtfully organized to minimize travel, typically dedicating one day to northern communities and the remaining days to southern communities.
Responsibilities

Conduct patient assessments and wellness visits in the home

Perform medication pre-pours and monitor patient status

Develop and update care plans

Coordinate services with patients, families, caregivers, and providers

Supervise CNA and LPN services as required

Identify changes in condition and communicate concerns appropriately

Support patients in maintaining independence and quality of life
Qualifications

Current Connecticut RN license
-
Home health, community health, chronic care, or case management experience preferred

Strong assessment and communication skills
-
Reliable transportation and willingness to travel throughout the service area

Current CPR certification
