Position Summary:
The Western Dakota Clinic RN provides professional nursing services by performing patient assessment, triage, care planning, and care coordination within a busy outpatient clinic environment. This role supports providers through rooming, clinical decision-making, patient education, and care management activities for individuals with both acute and chronic conditions. Working as part of a collaborative interdisciplinary team, the Clinic RN contributes to safe, timely, and evidence-based care while helping ensure efficient clinic flow and clear communication. This position fosters a supportive care setting where patients and families feel informed, respected, and actively engaged in their care plans.
Key Responsibilities:
- Perform comprehensive nursing assessments, triage patients in person and by phone, and collaborate with providers on care plans.
- Administer medications, treatments, and immunizations safely and accurately in accordance with provider orders and nursing standards.
- Provide patient and family education, care coordination, and support for chronic disease management and preventive care.
- Use the electronic health record to document assessments, interventions, and communications clearly and timely.
- Participate in population health and outreach activities, including recall lists, care gap closure, and follow up on test results.
- Support quality improvement, safety initiatives, and clinic flow optimization through active participation in team meetings and projects.
