RN Coordinator

Job Purpose:
- The RN Coordinator at Neighborhood Family Practice is responsible for overseeing and coordinating patient care activities to ensure the delivery of high-quality healthcare services. This role involves collaborating with healthcare teams, managing patient care plans, and ensuring compliance with healthcare regulations and standards.

Key Responsibilities:
- Coordinate and oversee patient care activities to ensure efficient and effective service delivery.
- Collaborate with healthcare teams to develop, implement, and evaluate patient care plans.
- Monitor patient progress and adjust care plans as necessary to meet patient needs.
- Ensure compliance with healthcare regulations and standards in all patient care activities.
- Serve as a liaison between patients, families, and healthcare providers to facilitate communication and understanding.
- Provide guidance and support to nursing staff and other healthcare professionals.
- Participate in quality improvement initiatives to enhance patient care outcomes.
- Maintain accurate and up-to-date patient records and documentation.
- Assist in the development and implementation of policies and procedures related to patient care coordination.
