JOB TITLE: Diabetes Educator (RN)

DEPARTMENT: OPC Specialty Clinic

HOURS & SHIFT REQUIREMENTS: Full time, 80 hours a pay period, Monday – Friday with an occasional weekend or evening for community events.

GENERAL SUMMARY:

The Diabetes Care & Education Specialist provides education, support, and guidance to individuals with diabetes and their families to promote optimal self-management and improve health outcomes. This role involves assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure comprehensive diabetes care.

PRINCIPLE DUTIES AND RESPONSIBILITIES

Conduct assessments of patients’ knowledge, skills, and readiness for diabetes self-management.

Develop and deliver individualized diabetes education covering topics such as nutrition, blood glucose monitoring, medication management, diabetes-related technologies, lifestyle modification, and complication prevention.

Provide ongoing support, counseling, and follow-up to encourage adherence and behavioral changes.

Monitor patient progress and adjust education plans as needed.

Coordinate centralized continuous glucose monitoring (CGM) workflows, including device onboarding, data integration, and patient follow-up.

Provide remote patient monitoring of CGM data to identify trends, support timely interventions, and communicate recommendations to the care team.

Serve as a resource for insulin pump and automated insulin delivery (AID) systems, including patient education, troubleshooting, and therapy optimization.

Collaborate with healthcare providers, including physicians, nurses, dietitians, and pharmacists, to ensure coordinated care.

Maintain accurate documentation of education sessions and patient outcomes in accordance with regulatory standards.

Stay current with advances in diabetes care, treatment guidelines, and patient education strategies.

Participate in community outreach, workshops, and support groups as needed.

PHYSICAL REQUIREMENTS

Excellent, visual, hearing, speech/articulation abilities. Manual dexterity to use computer to communicate, investigate and educate. Work requires ability to sit at a computer for long periods of time (2-3 hours)


REPORTING RELATIONSHIP

The DCES reports directly to the DSMES Quality Coordinator.
