🌿 What You’ll Do

As an RN Case Manager, you’ll coordinate and oversee the interdisciplinary plan of care for an assigned caseload of hospice patients, ensuring they receive compassionate, high-quality care wherever they call home.

You’ll serve as a key clinical partner for patients, families, physicians, and the interdisciplinary hospice team throughout the patient’s hospice journey.

🩺 Key Responsibilities

• 💙 Manage an assigned caseload of hospice patients and families.

• 🩺 Complete comprehensive patient assessments and ongoing evaluations.

• 📋 Develop, implement, and update individualized plans of care.

• 💊 Support effective pain and symptom management.

• 🤝 Coordinate care with physicians and members of the interdisciplinary team.

• 🕊️ Provide compassionate education, guidance, and emotional support to patients and families.

• 👩‍⚕️ Supervise Hospice Aides and Licensed Practical Nurses as appropriate.

• 💻 Maintain accurate, complete, and timely electronic medical record documentation.

• 📞 Communicate changes in patient condition and care needs to the appropriate clinical team members.

• 🌱 Participate in interdisciplinary meetings and quality improvement initiatives.

• 🏠 Provide patient-centered hospice care wherever patients call home.

• ⭐ Deliver exceptional end-of-life care while upholding Big Bend Hospice’s mission and values.
