Job Summary and Responsibilities

As our RN Navigator, you will serve as the primary point of contact, coordinating care and services for identified patient populations across the continuum, and promoting effective resource utilization.

Every day you will assume a leadership role within the multidisciplinary team to identify and eliminate barriers to care, ensuring optimal clinical outcomes for patients and their caregivers.

To be successful in this role, you will be a Registered Nurse with strong clinical assessment and care coordination skills, a clear understanding of patient navigation, and the ability to effectively collaborate with multidisciplinary teams to eliminate barriers and optimize patient outcomes.

  • Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the patient from first contact and throughout the continuum of care.
  • Guide patients on treatment plans, next steps in appointments (i.e., surgical prep, imaging, labs, tests, which provider to see next, etc.).
  • Assist with scheduling followup appointments and provide information and reminders to patients & physicians’ offices.
  • Educates the patient and family on care specific guidelines and treatment options to enable patient-led treatment decisions.
  • Educates and facilitates patient on introduction to program clinical trial information, resource referrals, and counseling.
  • Advocates for patients and families in preventing/resolving clinical issues; develops supportive unit practices and guidelines.
  • Develops strong relationships with physicians, their office staff, community resources and healthcare providers to assure effective communication in patient care.
  • Serves as a liaison between clinical specialists, primary care physicians and patients.

Job Requirements

Required

  • Bachelor's Degree in Nursing
  • Three (3) years of experience, with one (1) year in specialty.
  • Strong psychosocial assessment and clinical skills.
  • RN License; Current state nursing licenses or valid RN license from a participating state in the National Licensure Compact.
  • Current Basic Life Support through the American Heart Association.

Preferred

  • Nursing Professional Certification
