AMI-Wellness Home Health is seeking an experienced RN to provide part-time skilled nursing visits for home health patients in the Denver metro area.
This is a per-visit position, generally scheduled Monday through Friday based on patient needs, referral volume, and nurse availability. This is not a full-time position.
The RN will support home health patients through Starts of Care, Resumptions of Care, Readmissions, and skilled nursing revisits as assigned.
Responsibilities
- Complete skilled nursing visits in patient homes
- Perform Starts of Care, Resumptions of Care, Readmissions, and follow-up visits as assigned
- Assess patient condition and communicate changes appropriately
- Provide patient education, medication review, wound care/assessment, and other skilled nursing services within RN scope of practice
- Communicate with physicians, supervisors, patients, families, caregivers, and the care team
- Complete timely and accurate electronic documentation
- Support safe transitions into home health care
Schedule and Compensation
- Part-time, per-visit position
- Visits generally completed Monday through Friday
- Paid per visit, with rates varying by visit type
- Flexible weekday field nursing opportunity without a full-time schedule
