RN Registered Nurse (Fitchburg/Leominster Area)

Care Central VNA & Hospice is looking for a Registered Nurse that will be responsible for nursing assessment of clients in the Leominster/Fitchburg area.

Responsibilities include, but are not limited to:

• Responsible for nursing assessment of clients in the home.
• Developing a plan of care (POC) with the physician, client, family, updating the POC, and changing the POC as needed by ongoing intermittent assessment and evaluation.
• Plans for coordinated nursing service for individuals and families.
• Appraises individual and family health needs and hazards (existing or potential).
• Provides intermittent nursing care to clients as well as teaching and supervision.
• Consults with and refers families to appropriate personnel within the Agency or other community resources.

1. Education: Graduate of a N.L.N. approved school of nursing.
2. Experience: One-year nursing experience in an acute care setting or comparable experience is recommended. Experience in community health nursing preferred.
3. Licensure: Valid, unencumbered license in the State of Massachusetts. Possession of a valid driver’s license and dependable, registered transportation

1. Other: Basic knowledge of computer use, adequate typing skills and spelling skills. Ability to read, write, speak and understand the English language.
