⚕️ What You’ll Do
- Complete Start of Care (SOC) assessments in accordance with state and agency guidelines
- Perform skilled nursing visits, including wound care, medication management, patient education, and chronic disease monitoring
- Develop and update individualized care plans
- Communicate with physicians, case managers, and the interdisciplinary team
- Document accurately and timely in the EMR
- Provide high-quality, compassionate care to patients in their homes
✅ Requirements
- Active Florida RN or LPN license
- Current CPR certification
- Compassion, patience, and a sense of humor (seriously — it helps!)
- TB skin test (current within the last 12 months)
- Six months of prior hands-on nursing experience preferred but not required
- Experience with pediatric or adult high-acuity care is a plus — but we’ll train the right nurse!
- Must have reliable transportation
