Roads to Recovery - Case Manager II
Position Overview:
As a Case Manager II at a in-custody (jails) program , your role will involve providing comprehensive support services to individuals who are incarcerated, You will be responsible for assessing participants' needs, developing individualized service plans, and connecting them with appropriate resources to help them regain stability and move towards permanent housing. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency.
Duties and Responsibilities:
- Client Assessment: Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals.
- Case Planning: Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs, such as housing, employment, healthcare, and social support.
- Referrals and Resources: Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance.
- Advocacy: Advocate for clients' rights and needs within the shelter and in the community, including liaising with social service agencies, government entities, and community organizations.
- Crisis Intervention: Provide immediate support and intervention during crisis situations, such as mental health crises, substance abuse relapses, domestic violence incidents, or medical emergencies.
- Case Monitoring and Documentation: Regularly monitor clients' progress, document interactions, and maintain accurate and confidential case records. Update case plans as necessary based on clients' changing needs.
- Supportive Counseling: Offer emotional support, guidance, and counseling to clients, helping them develop coping skills, problem-solving abilities, and positive relationships.
- Life Skills Training: Facilitate workshops and group sessions on life skills, including budgeting, job readiness, communication skills, and conflict resolution.
- Collaboration: Collaborate with other case managers, shelter staff, and external service providers to ensure coordinated and holistic care for clients.
- Program Evaluation: Assist in evaluating the effectiveness of the shelter's programs and services, providing feedback and recommendations for improvement.
- Professional Development: Stay updated on best practices in case management, homelessness, and related fields through ongoing training and professional development opportunities.
