Salary Range: $25/hr - $30.55/hr plus an Elite Benefit Package
Hours: 40 hours/week (Mon-Fri, 8:30a-4:30p, flexibility may be required for meetings)
Location: Latham office/Hybrid WFH after first 60 days

Summary
A Self Directed Services Coordinator drives operational, financial, and regulatory integrity for a 80-person caseload in OPWDD Self-Directed Services—empowering individuals to maximize their independence while maintaining compliance with state and federal healthcare standards.
Education/Experience:
- Bachelor's Degree in a Human Services-related field preferred.
- This may be substituted with an Associates’ Degree and one-year of experience in working with people with Intellectual and Developmental Disabilities.
- QIDP Preferred

Essential Duties and Responsibilities include the following: (Other duties may be assigned)
- Overseeing a caseload of up to 80 individuals. This includes:
- Processing all reimbursements for individuals, vendors, and staff; and ensuring that reimbursements meet all Medicaid and OPWDD requirements.
- Reviewing all staff notes on a monthly basis
- Monitoring expenses and reviewing monthly expenditure reports and sharing monthly reports to each individual’s team.
- Acting as the main point of contact for the individuals’ self-directed services.
- Attending biannual Life Plan meetings for all individuals and reviewing Life Plans to ensure all Medicaid and OPWDD requirements for service delivery and billing are met
- Reviewing Staff Action Plans created by Support Brokers to ensure they match the Life Plan
- Communicating with various Wildwood departments involved in a person’s Self-Direction services (HR,
Finance, Training). - Maintaining annual required OPWDD and Wildwood trainings
- Ensuring all documentation is scanned and saved for a period of 10 years, regular auditing of individual’s
files, and maintaining organization of electronic files. - Keeping up on OPWDD and Medicaid regulations as they pertain to Self-Direction services, and
communicating these regulations to participants as needed. - Compiling information for all individuals on their caseload who receive a Housing Subsidy; this may include doing Quality Assurance walkthroughs, home visits, assisting in gathering documents, and completing monthly progress notes for billing of services.
