Senior Case Manager

Position Summary:

Person in this position will be responsible for supervising the day-to-day functions of the STOP Program. Essential Job Functions include those listed below. Other duties may be assigned as needed

Essential Job Functions:

• Supervision of the case management of a large caseload of participants released from prison substance abuse programs.
• Auditing all active participant files to ensure all files are complete according to WestCare guidelines.
• Coordination of various services throughout several agencies.
• In charge of scheduling site visits and institutional presentations.
• Supervision of Community Service Coordinators (Case Managers) including accompanying them on site visits and holding weekly staff meetings.
• Prepare any necessary reports.
• Travel to community-based providers.
• Responsible for hiring, evaluating, and training of STOP staff under the direction of the Director.
• Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
• Perform any other duties as assigned.
