This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

HUD-VASH is an intensive case management program for chronically homeless Veterans, aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The incumbent in this position functions as the Medical Center's Liaison/Case Manager for Fresno VAMC's Healthcare for Homeless Veterans Program Department of Housing and Urban Development / VA Supportive Housing (HUD-VASH). The incumbent is responsible for monitoring all phases of programming and coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional - The incumbent is responsible for overseeing all aspects of the HUD-VASH program. - The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements. - The incumbent performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. - The incumbent verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. - The incumbent is responsible for establishing a process for referrals and admissions to the HUD-VASH Program, ensuring that homeless Veterans are considered as primary participants for program acceptance. - The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. - The incumbent is responsible for providing oversight of HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. - The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, linking, and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. - The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager. - The incumbent actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH. - The incumbent is responsible for developing and administering local policy and SOP for the HUDVASH - Program. This includes maintaining policies and documentation as required for CARF and JCAHO accreditation.
