Service Coordinator

Summary

Coordinates care plans, referrals, and support for clients with disabilities, ensuring compliance and person-centered services while maintaining documentation and community partnerships.

The Service Coordinator plays an essential role in supporting individuals and families served by WCDC, Inc. This position ensures that clients have access to coordinated, person-centered, and culturally responsive services that promote stability, independence, and well-being.

DUTIES/RESPONSIBILITIES:

  • Ensure compliance with safety & person-centered support plans when executing programming needs of the clients.
  • Conduct intake and assessments to determine client needs, strengths, and goals. Update assessments as appropriate.
  • Develop individualized service or care plans and support clients in achieving their goals.
  • Coordinate services and referrals with community partners, agencies, and internal programs.
  • Provide ongoing follow-up, advocacy, and support to ensure successful connection to resources.
  • Maintain accurate, timely, and confidential client documentation in accordance with agency and funding requirements.
  • Work collaboratively with internal and external team members to promote communication, service integration, and positive outcomes.
  • Crisis intervention and handling of client disciplinary issues and behavior modification for inappropriate behaviors.

  • Attend regular team, client advisory, client staffings & internal department meetings.
  • Participate in company committees and attend training and seminars to continue developing professional skills.
  • Maintain professional development and continuing education requirements as applicable to role and licensure.
  • Represent WCDC, Inc. and uphold its mission, vision, and values in all community and client interactions.
  • Maintain a positive working relationship with other personnel, volunteers and consumers.
  • Notify the Director of Day Programming of client complaints regarding the program or staff and act upon complaints per grievance policy and procedures.
  • Ensure timely completion of internal and external incident reporting including dependent adult and child abuse reporting.
  • Complete Client Satisfaction Surveys annually with clients and/or Guardians.
  • Provide activity and floor coverage for breaks and staff absences when appropriate.
  • Supervise social work students in fulfilling service coordinator experience practicum requirements. Provide feedback to student practicum supervisors when requested.
  • Assist President & management team with identifying opportunities for service expansion, including but not limited to residential and hourly supported community living (SCL), adult day habilitation and supported employment programming.

Pay based on education and experience
