Service Line Director Family Practice
BASIC FUNCTION
The Service Line Director – Family Practice is responsible for the strategic, operational, and financial leadership of HealthPoint’s Family Practice service line across multiple clinic sites. This role ensures high-quality, patient-centered care while advancing productivity, provider engagement, community partnerships, and financial sustainability in alignment with HealthPoint’s mission as a Federally Qualified Health Center (FQHC).
The Director works collaboratively with clinical leadership, operations, finance, quality, and community stakeholders to strengthen access, optimize performance, and support the growth of family practice services across rural communities.
PRIMARY RESPONSIBILITIES AND DUTIES
- Strategic Planning & Service Line Development
- Lead the long-term vision, strategy, and growth of HealthPoint’s Family Practice service line across all assigned clinics, ensuring alignment with organizational mission, strategic priorities, and community health needs.
- Develop, implement, and routinely update a comprehensive service line strategic plan that addresses access to care, care model design, workforce planning, quality outcomes, and financial sustainability.
- Evaluate service line performance using clinical, operational, financial, and population health data to identify gaps, opportunities, and risks.
- Identify and advance opportunities for service expansion, including new clinic sites, extended hours, innovative care delivery models (team-based care, integrated services), and telehealth where appropriate.
- Collaborate with executive leadership on new program development, grant opportunities, and initiatives that strengthen primary care delivery in rural and underserved communities.
- Productivity & Performance Management
- Oversee clinic and provider productivity across the Family Practice service line, including visit volume, provider FTE utilization, access metrics, panel size, no-show rates, and patient throughput.
- Establish clear performance expectations and benchmarks in collaboration with medical leadership, operations, and finance.
- Monitor productivity and performance dashboards regularly; proactively address underperformance through data analysis, workflow redesign, staffing adjustments, and operational improvements.
- Partner with clinic leadership to optimize scheduling templates, staffing ratios, room utilization, and care team roles to improve efficiency while maintaining quality and patient satisfaction.
- Support continuous performance improvement initiatives focused on access, continuity of care, patient experience, and outcomes.
- Provider Relations & Clinical Collaboration
- Serve as a trusted leader and advocate for family practice providers, fostering strong relationships built on transparency, collaboration, and shared accountability.
- Work closely with Medical Directors and clinical leadership to ensure alignment between operational goals and clinical practice standards.
- Support provider recruitment, onboarding, retention, and engagement in partnership with Human Resources and Medical Leadership.
- Facilitate provider input into operational decisions, workflow design, and service line strategy.
- Address provider concerns related to productivity expectations, resources, workflows, and work-life balance in a timely and constructive manner.
- Support provider development and leadership growth, including participation in quality initiatives, care model redesign, and organizational committees.
- Community Engagement & Partnerships
- Represent HealthPoint and the Family Practice service line in the communities served, strengthening HealthPoint’s presence and reputation as a trusted primary care provider.
- Build and maintain strategic relationships with hospitals, specialty providers, behavioral health partners, schools, long-term care facilities, social service agencies, and community organizations.
- Identify partnership opportunities that enhance care coordination, referral management, population health outcomes, and access to services for patients.
- Collaborate with outreach, marketing, and care coordination teams to align community engagement activities with service line goals.
- Participate in community meetings, advisory boards, and collaborative initiatives to support population health improvement and address social determinants of health.
- Financial Management & Clinic Oversight
- Provide financial leadership for the Family Practice service line, including support to the finance department on the development and management of annual budgets, forecasts, and long-range financial plans.
- Monitor and analyze service line financial performance, including revenue, expenses, payer mix, cost per visit, and contribution margin.
- Partner with finance, billing, and coding teams to optimize documentation, coding accuracy, reimbursement, and revenue cycle performance.
- Identify opportunities to improve financial performance while maintaining HealthPoint’s mission and commitment to underserved populations.
- Support compliance with FQHC financial requirements, HRSA expectations, and audit processes.
- Develop and implement corrective action plans when clinics or the service line are not meeting financial or operational targets.
- Quality, Compliance & Mission Alignment
- Ensure Family Practice services consistently meet clinical quality standards, patient safety requirements, and regulatory expectations.
- Collaborate with Quality, Compliance, and Clinical Informatics teams to support UDS measures, quality improvement initiatives, and reporting requirements.
- Promote a culture of continuous improvement, accountability, and patient-centered care across the service line.
- Ensure all service line activities align with HealthPoint’s mission, values, and commitment to equitable access to high-quality care.
