Services Coordinator
A Service Coordinator helps organize, coordinate, and monitor services for children with developmental delays, disabilities, or special needs and supports their families in accessing appropriate care and resources
- Coordinate developmental assessment appointments and therapy services.
- Act as the primary liaison between families, healthcare providers, therapists, schools, and community agencies.
- Conduct intake interviews and gather relevant medical, developmental, and social information.
- Develop, implement, and monitor individualized service plans.
- Schedule appointments and maintain service calendars.
- Ensure timely referrals to specialists and community support programs.
- Maintain accurate and confidential client records and documentation.
- Monitor service delivery and follow up on missed appointments or unmet needs.
- Educate families about available services, benefits, and community resources.
- Assist families with accessing financial assistance or government support programs when applicable.
- Participate in multidisciplinary team meetings and case conferences.
- Prepare reports, statistics, and documentation for quality assurance and compliance.
- Ensure compliance with organizational policies, child safeguarding standards, and applicable healthcare regulations.
- Support continuous quality improvement initiatives within the department.
- Bachelor's degree in Social Work, Psychology, Child Development, Nursing, Healthcare Administration, Education, or a related field.
- Relevant experience (typically 2–5 years) in child development, healthcare coordination, rehabilitation, or social services.
- Knowledge of child development milestones and developmental disorders.
- Familiarity with multidisciplinary healthcare or rehabilitation services.
