Key Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.

• Assist residents in improving strength, mobility, balance, and functional independence.

• Implement individualized treatment plans established by the supervising Physical Therapist.

• Monitor resident progress and communicate changes to the therapy team.

• Maintain accurate and timely documentation in accordance with facility and regulatory requirements.

• Collaborate with nursing, therapy, and interdisciplinary teams to support positive resident outcomes.

• Educate residents and families on therapeutic exercises, mobility techniques, and safety.

Qualifications

• Active Arizona Physical Therapist Assistant (PTA) license required.

• Graduate of an accredited Physical Therapist Assistant program.

• Previous skilled nursing, long-term care, or rehabilitation experience preferred.

• New graduates are welcome to apply.

• Strong communication, organizational, and documentation skills.

• Ability to work independently and collaboratively in a team environment.
