Provides nutrition counseling and support services to LGBTQ+ youth, documenting services in the SCN IT Platform to ensure Medicaid compliance.
The Ali Forney Center (AFC) protects LGBTQ+ youth from homelessness and helps them build the tools needed to live independently. Founded in 2002, AFC provides housing, healthcare, and educational support to thousands of youth each year. AFC creates an affirming, supportive environment where LGBTQ+ youth can thrive.
The Social Care Network (SCN) Registered Dietitian & Nutritionistprovides nutrition counseling, education, and support services as part of the Ali Forney Center’s participation in New York State’s Medicaid 1115 Waiver Social Care Network initiative. This role supports AFC’s ability to deliver and bill for eligible nutrition-related HRSN services, including nutrition counseling and pantry stocking supports.
The Registered Dietitian & Nutritionist works directly with LGBTQ+ youth and young adults experiencing housing instability, food insecurity, chronic health conditions, and barriers to wellness. The position supports individualized nutrition planning, healthy meal education, and coordination of medically appropriate food resources while ensuring services are documented in compliance with SCN requirements.
Key Responsibilities
- Provide nutrition counseling and education services to eligible clients enrolled in SCN services.
- Conduct nutrition assessments and develop individualized nutrition support plans.
- Educate clients on healthy eating, meal preparation, grocery budgeting, and food access resources.
- Support implementation of food security initiatives including pantry stocking and nutrition-related referrals.
- Document services accurately and timely within the SCN IT Platform and other required systems.
- Collaborate with case managers, navigators, housing staff, and healthcare providers to coordinate holistic care plans.
- Ensure compliance with Medicaid, SCN, and NYSDOH documentation and billing requirements.
- Assist with program reporting, audits, quality assurance, and reimbursement readiness.
- Participate in interdisciplinary team meetings and case conferences.
- Support culturally responsive and trauma-informed nutrition services for LGBTQ+ youth populations.
