About the Position: This is an open continuous announcement. Multiple vacancies exist and may be filled from this announcement at any of the locations listed. This is a Direct Hire Solicitation Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes.

Provide mental health initial screening and follow-up care for a high-risk population in a collaborative team approach.

Conduct initial patient interview to collect factual information and assess the needs of the patient.

Perform direct patient support through patient referrals and education.

Assist provider(s) in the development of criteria for standards of care and patient outcomes.

Operate computer to input data and monitor reports.

Maintain ongoing follow-up with patients identified in caseload.
