Social Services Assistant

About the Position: This position is located at Fort Riley, Kansas. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes. This is a Direct Hire Solicitation

Provide mental health initial screening and follow-up care for a high-risk population in a collaborative team approach.

Conduct initial patient interview to collect factual information and assess the needs of the patient.

Develop support caseload for the Social Workers and clinical mental health providers

Maintain ongoing follow-up with patients.

Perform direct patient support through patient referrals and education.

Assist provider(s) in the development of criteria for standards of care and patient outcomes.

Operate computer to input data and monitor reports.
