Social Services Director

Darby Glenn, a skilled nursing and rehabilitation facility located in Hilliard, OH is seeking a strong and dedicated Full Time Social Services Director to join our dynamic management team.

The Social Worker is responsible for managing and/or assisting with various resident services including MDS, PASSAR requirements, Ancillary Services, Level of Care (LOC) and Medicaid eligibility requirements as well as discharge coordination, care conferences and assisting with quality assurance for resident services.

The successful candidate will have previous experience in a long-term care environment and exhibit a compassionate and empathetic demeanor in caring for our residents.
