As a Social Work Associate, you will be responsible for the following duties:
Perform assessment in financial application cases
Assisting patients in national financial schemes application
Assist medical social worker in reviewing assessing cases
Work together with medical social worker in providing brief social assessment
Collecting and organizing information for case assessment
Perform home visits when necessary
Educating patients and family members
Act as mentors to new Social Work Assistant within the team
Participate in quality improvement projects when necessary
Any other duties within the team
Requirements:
Min. Diploma in Social Work or equivalent.
