- Salary: Up to S$3300 (commensurate with experience)
- 5 Day Work Week: Mondays –Fridays
- Location: South Central and West
- Employment Type: Full-Time, Permanent
A leading healthcare and eldercare organisation is seeking a compassionate Social Work Associate to join its growing team.
Job Responsibilities:
- Manage case referrals
- Consult with the centre lead to conduct assessment, planning and intervention for cases
- Monitor the delivery of coordinated services
- Adopt a suitable case management approach when working with clients
- Refer and coordinate care services to ensure clients are adequately supported
- Keep abreast of, and provide information on, the various assistance schemes and care options available
- Assess family financial status
- Manage the administration of grants, subsidies and support services where relevant
- Ensure all MOH service requirements on financial and quality aspects are adhered to
Job Requirements:
- Bachelor’s Degree, preferably in Social Work
- Degrees in Counselling, Gerontology, Psychology, Sociology or related disciplines are also welcome
- Min. 1 year of experience in social work, preferably in community or healthcare settings
- Strong communication and stakeholder management skills
- Proficient in Google Suite applications
