Social Work Manager

Job Description

We are seeking a compassionate and committed Medical Social Worker to join our nursing home team. The successful candidate will provide psychosocial support, case management and care coordination for elderly residents and their families.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide counselling and emotional support to residents and their families.
• Assist residents and families with financial assistance and social-service resources.
• Facilitate family meetings, case conferences and care-planning discussions.
• Coordinate with hospitals, community agencies and other healthcare providers.
• Support admission, discharge planning and continuity of care.
• Identify and manage psychosocial, family, safeguarding and end-of-life concerns.
• Maintain accurate case documentation and ensure confidentiality.
• Work collaboratively with the multidisciplinary healthcare team.
• Participate in service improvement and other relevant projects.
• Liaising and sourcing for donation items and volunteer management.

Requirements

• Degree in Social Work from a recognized institution.
• Experience in healthcare, eldercare, nursing homes or community social services is an advantage.
• Strong counselling, communication, assessment and case-management skills.
• Good knowledge of Singapore's healthcare and social-service resources.
• Compassionate, empathetic and able to work effectively with elderly residents and their families.
• Able to work independently as well as within a multidisciplinary team.
