Social Worker Associate

Job purpose

To facilitate holistic support for clients receiving rehabilitation services by coordinating relevant services and community resources, assisting with financial and social support needs, and connecting clients with appropriate interest groups and activities to promote quality of life and community participation.

Duties and responsibilities

Admissions and Records:

• Coordinate client referrals, intake, and admission processes.
• Assess clients’ social and support needs and facilitate appropriate services.
• Link clients to community resources, special interest groups, and social activities.
• Support clients in enhancing social participation and quality of life.
• Coordinate and follow up on referrals to social, healthcare, and community services.

Billing and Subsidies:

• Manage FA and FD applications
• Manage and update subsidy reports (NMTS)
• Assist clients with financial assistance, subsidies, and relevant support schemes.

General Administration:

• Maintain accurate client records and documentation.
• Liaise with clients, caregivers, rehabilitation team, and external agencies.
• Perform other administrative and social work support duties as required.

Qualifications & Skills Required

• Diploma or GCE O-Levels with relevant social work experience.
• Proficiency in office software and strong interpersonal communication.
• Familiarity with eldercare, intermediate and long-term care (ILTC) systems, NMTS and BRIGHT system is an advantage.

Other details

• Mon to Fri, 8am to 6pm (44 hours per week)
• West & Centre area (required to travel between different centres)
