- Support Complex Needs: Serve as a frontline resource to assess, navigate, and support legal, substance abuse, crisis, and psychosocial challenges.
- Advocate for Patients: Proactively remove barriers to care and connect individuals with critical community resources.
- Collaborate for Continuity: Maintain continuous dialogue with interdisciplinary care teams to ensure seamless transitions and responsive health plans.
- Inform & Document: Help shape post-discharge strategies and ensure precise documentation to align treatment options with patient needs.
- Required Credential(s):
- One or more of the following:
- Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- One or more of the following:
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
