Social Worker

- Mon-Fri; Day Shift
Provide evidence-based mental health counseling, psychosocial assessments, and individualized treatment planning.
Coordinate patient care with psychiatrists, interdisciplinary teams, and community agencies.
Maintain compliant EHR documentation, promote patient safety and quality improvement, and support staff education.
- Social Worker must have or be eligible to sit for LISW in the state of Iowa
- Must have a Master's Degree or higher in social work
- Previous outpatient or acute care experience preferred
