Social Worker MSW - MCC

Provides professional skills and advanced social services. Responsibilities include performing extended social work program development, case management, clinical research, outreach, patient assessment and evaluation requiring advanced skills and training.

• Performs psychosocial assessments, health education, supportive counseling and referrals; provides education on social services and public systems; assists patients in finding resources.
• Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.
• Identifies obstacles to optimal patient care; loss or grief work; risk reduction; and health promotion.
• Provides clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified.
• Formulates candid and supportive communication.
• Coordinates ongoing counseling and support groups.
• Facilitates effective problem solving and conflict resolution.
• Participates in program evaluation activities including but not limited to data collection for grants, grant reporting, patient satisfaction surveys, and other activities as needed.
• Performs all other duties as assigned.

• Excellent communication skills, both verbally and written; bilingual (Spanish/English) preferred.
• Ability to work collaboratively and flexibly with a diverse team.
• Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
• Knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
• Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures.
• Independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
• Experience using Microsoft Office (Word, Excel, etc.) and data management software.

EDUCATION:

Master's degree in Social Work is required.

EXPERIENCE:

Three (3) years of professional social work experience is required.

LICENSES/CERTIFICATIONS:

License as a Social Worker in Texas (LMSW, LSW) is required.
