Position Summary
This social work position will support Physical Medicine and Rehabilitation, specifically stroke clinic, traumatic brain injury clinic and Cerebral Palsy Clinic. This position will be primarily outpatient focused, but there will be opportunity to provide continuity of care and support patients who are admitted. This position focuses on care coordination for complex patients with multiple medical conditions and psychosocial needs.
Fosters positive and professional interpersonal relationships with patients/families, physicians, hospital staff, community agency representatives and visitors. Provides comprehensive psychosocial assessment for identified patients and based on findings, facilitates interventions. Effectively uses interpersonal communication skills to resolve conflicts and promote healing environment.
