Make a meaningful impact by helping patients and families navigate the hospice journey with compassion, guidance, and support. As a Social Worker with Big Bend Hospice, you'll collaborate closely with our interdisciplinary team to ensure patients and caregivers receive the emotional, educational, and community resources they need.

🌟 What You'll Do

✅ Complete psychosocial and bereavement assessments for new admissions
✅ Provide counseling, emotional support, and crisis intervention for patients and families
✅ Educate patients and caregivers on end-of-life care, advance directives, long-term care planning, and available resources
✅ Assist with financial assessments and Medicaid application support
✅ Connect patients and families with community resources and services
✅ Manage a caseload of approximately 30-35 patients while maintaining timely documentation 📝
✅ Participate in Interdisciplinary Team (IDT) meetings and collaborative care planning 🤝
✅ Conduct visits in homes, facilities, hospitals, and hospice units 🚗
✅ Participate in on-call coverage every 6-8 weeks 📞
