This position is for a Social Work Program Coordinator (Care Coordination & Integrated Case Management), at the Aleda E Lutz Medical Center in Saginaw, Michigan. The Program manager provides both clinical and administrative leadership, ensuring the compliance with local facility, VISN, Office of Nursing Service (ONS), Care Management and Social Work (CMSW),

Total Rewards of a Allied Health Professional Duties may include but are not limited to: The Social Work CC&ICM Program Coordinator provides clinical leadership in the development and implementation of facility-level processes consistent with the national CC&ICM framework, ensuring Veterans experience seamless transitions of care across settings. Implementing standardized CC&ICM tools and processes to integrate care horizontally and vertically (medical/mental health, inpatient/outpatient, VA/community). Identifying and monitoring cohorts of Veterans with the highest need and greatest opportunity for coordinated care. Integrating the work of inpatient and outpatient care coordinators, case managers, and navigators for seamless continuity. Developing, monitoring, and sustaining screening processes to identify Veterans appropriate for CC&ICM services. Coordinating and co-facilitating a Care Coordination Review Team (CCRT) to assign lead coordinators and ensure interdisciplinary collaboration. Serve as the subject matter expert for case management services, providing consultation and clinical guidance to less experienced staU. Collaborating with physicians, nurses, psychologists, pharmacists, and other disciplines to ensure coordinated, patient-centered care planning. Supporting and facilitating education of staff, leaders, and Veterans on the CC&ICM framework. Analyzing and evaluating program data to identify trends, gaps, and opportunities for quality improvement in care coordination. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
