Social Worker
- Conduct formal intake interviews; make assessments and referrals as needed.
- Complete psycho-socials and social history when applicable.
- Provide short and long-term counseling, crisis intervention, safety planning, and psycho-education.
- Maintain complete, accurate, and up-to-date programmatic paperwork and statistical information.
- Assist clients in completing entitlement forms as needed.
- Make referrals for various services, such as food, clothing, bathing, psychiatric evaluations, medical evaluations, TB and HIV testing, and accompany clients as needed and also handle follow-up as needed.
- Make referrals to other service areas within Project Hospitality and the community.
- Participate in weekly supervision, weekly staff meetings,
- Facilitate community meetings/groups and ongoing case conferences.
- Transport/Accompany clients to appropriate referral sites when deemed necessary.
