Social Worker - Social Service Care Manager

Catholic Housing & Community Services is currently seeking a full-time (40 hours/week) Social Service Care Manager for two older adult affordable housing sites located in Collingdale Borough, Delaware County and the Eastwick neighborhood in Philadelphia County.

As our Social Services Care Manager, you will be the heartbeat of our residential community and an integral part of redefining what it means to age in place. In this role, you are a strategic partner in our social service ecosystem, having the ability to engage and build relationships with older adults, access digital case management tools, participate in collaborative teams and build proactive and responsive social, recreational, and health-related services and programs in a relationship-centered, age affirming environment.

Primary responsibilities of this position include:

• Manage a caseload of older adult households at two independent apartment communities.

• Assess all residents for key strengths and barriers to successful aging in place, including physical and cognitive health/functioning, ability to manage daily living tasks, mobility, access to benefits and economic stability,

• Collaborate with residents to develop resident-centered care plans that focus on what matters to residents and identifies corresponding interventions and resources.

• Maintain up-to-date resident files in the social service database including timely case notes, key informal and formal support contacts, assessments, reassessments and care plans.

• Navigate and facilitate access to services and advocate with and on behalf of residents to community-based providers to minimize, mitigate and/or eliminate service delivery barriers.

• Create and maintain a community atmosphere that addresses holistic needs of the residents through onsite events and coordination with external providers. Events and gatherings requiring planning include: holiday and birthday parties, educational and health related presentations, and onsite vaccine clinic and community legal clinics.

• Maintain social work intervention and group participation data. Participate in and utilize data analysis tools to drive and improve program planning decisions.

• Respond to external phone calls for information and resources.

Position Requirements:

• Master's degree from an accredited university in Social Work, Gerontology or related field required.

• Minimum three (3) years' experience working with older adults.
