- Collaborate with medical teams, families and external agencies to guarantee patients receive fully supported and compliant treatment plans.
- Build strong relationships with community health and social organizations to connect clients with vital local resources.
- Guide the discharge planning process and coordinate post-care services to secure safe, seamless transitions for patients returning home.
Licensure / Certification / Registration
- Required Credential(s):
- None Required.
- Preferred Credential(s):
- Certification. Type 73 license through IL State Board of Education LBS I with endorsement in Transition preferred.
- Licensed Professional Counselor credentialed from the Illinois Department of Financial and Professional Regulation.
- Licensed Social Worker credentialed from the Illinois Department of Financial and Professional Regulation.
- Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation.
Education
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience
- 3 years of experience preferred.
